I understand change is never easy, so I wish to make this transition as stress-free as possible. If you have not already done so, I encourage you to explore our website to learn about the many academic, vocational, athletic, and artistic opportunities which await your child at South Fork High School. You will also be able to review the Florida graduation requirements which may differ from those of other states.

If, due to a loss of housing, you are forced to live in a shelter, motel, vehicle, or campgrounds, abandoned buildings, or doubled-up with relatives or friends; then you are eligible to receive services provided under the McKinney-Vento Act.

EACH STUDENT MUST FORMALLY APPLY AND BE APPROVED INTO A SIGNATURE PROGRAM.
Form 407 may be picked up at any Martin County School
Student must declare program at school of choice that they are applying to.
Student MUST remain in the approved program to continue enrollment in the choice school.
Student must be approved by program lead, principal and district
School Choice may be revoked for attendance or discipline problems.ALL CHOICE STUDENTS ARE RESPONSIBLE FOR THEIR OWN TRANSPORTATION TO THEIR CHOICE SCHOOL.
RECOMMENDATIONS ARE SUBJECT TO REVOCATION DUE TO THE CLASS SIZE REDUCTION STATE CONSTITUTIONAL AMENDMENT.
THE SCHOOL CHOICE APPROVAL PROCESS TAKES TIME. YOUR FAMILY WILL BE NOTIFIED VIA EMAIL OR LETTER WHEN A DECISION IS MADE. IN THE MEANTIME, IT WILL BE NECESSARY FOR YOUR STUDENT TO CONTINUE THE REGISTRATION PROCESS AT THEIR HOME SCHOOL.

Utility bill for service of residence (FPL, Cable, or Water) - In the parents/guardian's name.
Rental or Lease Agreement - In the parents/guardian's name
Contract to Purchase/build, or Mortgage Statement - in the name of thew parent/guardian
Notorized affidavit (form 90D) along with FPL bill of owner - if living with other family members or friends.
